SECTION 1. PARTICIPANT'S INFORMATION

Your name, organisation and email address will be printed in the conference proceedings booklet (with your abstract, if you are presenting) to facilitate staying in touch with other participants after the conference and letting you know about future APNME conferences. Abstracts and presenters' names and organisations (but not their email addresses) will also be published on the conference website. If you do not want your email address (or other details) to be included or used in this way please let us know by email to at the time of submitting this form.

Photographers and videographers are expected to be present during the conference and to take photos and videos which may be edited and used on the APNME website and in publications for promotional/record purposes.  If you do not wish your image to be used in this way please avoid prominent positions during the conference and let us know by email to at the time of submitting this form.

SECTION 2. CONFERENCE REGISTRATION FEE

Conference registration fees (whether or not a participant is presenting) are as follows (please select the appropriate button):

** Residents of Hong Kong/Macau should register under the International category
*** Eligibility for the student rate is at the discretion of the Conference Committee and dependent on production of a copy of a current student ID card issued by your institution at the time of registration. Please email a copy of your student ID card to at the time of submitting this form.

Note: Unless they have been informed otherwise on acceptance of their abstract, presenters must have registered and paid the conference fee by 15 September, at the latest, to ensure being included in the Conference Programme. If you have not registered and paid by that date then your presentation may be withdrawn from the Conference without further notice.

The registration fee covers the main conference activities including the following:

• Admission to all plenary, parallel, poster and other conference sessions
• One conference kit, including programme booklet with abstracts (proceedings)
• Welcome reception at Fudan University on 23 October 2014
• Welcome dinner at Crowne Plaza Shanghai Fudan Hotel on 24 October
• Lunches and coffee breaks on 24-26 October
• Dinner at Shanghai Normal University on 25 October
• School visit
• Eligibility to join Farewell dinner on 26 October (supplementary charges apply); see below
• Eligibility to join city tour on 27 October (supplementary charges apply); see below
• One copy of the conference group photograph

Please note that accommodation is not included.

Shuttle bus transportation is included for the school visit on 24 October (except for The High School Affiliated to Fudan University) and the conference sessions at Shanghai Normal University on 25 October. Transportation from the two recommended hotels is not being arranged as they are both within short walking distance of the conference venue at Fudan. Participants staying elsewhere should make their own transport arrangements to and from Fudan.

SECTION 3. ACCOMMODATION

Please note that the conference registration fee does not include accommodation. The Local Organising Committee recommends two hotels, both of which are offering conference-negotiated rates. These are:

• the Crowne Plaza Shanghai Fudan Hotel , which is about five minutes' walk from the main conference venue on Fudan's campus and offers international standard accommodation; and
• the Qingyun Guest House Hotel which is on the campus of the University and offers lower cost accommodation.

The reservation of and payment for a room at either of these hotels, or the making of preferred alternative accommodation arrangements, is the sole responsibility of each participant.

If you wish to stay at the Crowne Plaza Shanghai Fudan at the above conference room rate (which includes breakfast) please book your room in one or the other of the following ways:

• Book online through the Crowne Plaza website ,
  noting that you should see "APNME Conference" on the page where you select the room type and that the online form will not accept dates more than three days before/after the conference. Please also note that the rate quoted for a single room on the site, RMB 652.17, does not include the 15% service charge, which will increase the total payable to RMB 750 per night.

• Download the reservation form here and, once completed, send it directly to the Hotel by fax or email as indicated on the form.

N.B. If you experience difficulty in using the online facility and booking a room at the conference rate we suggest that you use the downloadable form .

Notes:

• Rooms at the conference rate are only available up to 22 September as the final deadline date and in any event are available on a first-come, first-served basis so you are encouraged to book your accommodation immediately after registering for the conference.  After the above date, neither room nor conference room rate availability is guaranteed.  Please book early!
• The conference room rate may not be available if, when booking, you do not use the above forms (or the above Crowne Plaza website, which includes conference coding).

SECTION 4. SCHOOL VISIT

After an early lunch on 24 October; participants have the option of visiting one of local three schools, about which there is information here . There is no additional cost for this activity, which is an integral part of the conference and will be followed by reflections at each school on the class observed. If you have a preference, please indicate below your 1st, 2nd and 3rd choices of school but note that we cannot undertake to accommodate your preference, in particular in the event that a school obtains too many registrants. If you do not indicate a preference, allocation will be at the Conference Committee's discretion.

SECTION 5. OPTIONAL EXTRA ACTIVITIES

The Conference registration fee does not include:

• The Farewell dinner at the Crowne Plaza Shanghai Fudan Hotel on 26 October; or
• City tour on 27 October.

Payment of conference fees and for optional activities should be made online at the time of submitting the online registration form.  Registration is payable at the full rate unless payment is received before the closing of the early-bird rate for conference registration, i.e. by 15 September (local time in Shanghai).

Please complete and submit this form and make your payment as soon as you can, and by 15 September 2014 at the latest if you are an intended presenter.

International conference fees are shown in US$ but the sum to be charged by Fudan's bank/payment agent in China will be the RMB equivalent of the US$ amount shown. Unless your card is billed to you in RMB, your bank or credit card company will exchange the RMB amount into your local currency and so the sum charged to your credit card account (even if in US$) may be slightly different from the US$ amount submitted by you when registering.

Cancellation Policy
In the unfortunate event that you wish to cancel a booking that you have made, please do so by email to the Conference Secretariat at . If notification is received in Shanghai by 24 September 2014, a refund of 50% will be made. We regret that no refund can be made for cancellations after 24 September 2014.

Receipt
You will receive a receipt from Fudan University in due course. Please let us know at if you have not received a receipt within two weeks of making payment.
